(Insert name) has been my patient for the past several years and in that time her weight has increased steadily despite genuine weight loss attempts. Her obesity is LIFE THREATENING.

It is my opinion that the Lap Band surgery (insert name) has requested will improve the quality of her life. Currently she suffers from and is taking medication for an anxiety disorder and hypertension. She also suffers from chronic back pain. She was diagnosed with PCOS several years ago and losing weight via the Lap Band surgery will greatly aid in the reduction of symptoms.

The Lap-Band surgery IS NOT READILY AVAILABLE THROUGH THE PUBLIC SYSTEM.

The Lap Band surgery is NOT READILY AVAILABLE THROUGH THE PUBLIC HEALTH SYSTEM and I would support her application for the funds to be made available from her superannuation.
Should you require any further information, then I would be happy to forward a medical report following a signed application for the release of confidential information by the patient.

(Insert patient's name) has been my patient for (insert how long) and in that time her weight has increased steadily despite genuine weight loss attempts. Her B.M.I. is 51 putting her in the Morbid Obesity Range and at this level her obesity is LIFE THREATENING. As a consequence of the obesity she is experiencing daily difficulties in carrying out her employment duties as a nurse. She has requested Lap Band surgery and it is my opinion that her weight loss as a result of the Lap Band surgery will improve her health dramatically reducing the life threatening risk to her health, enable her to carry out her employment duties without difficulties and improve her quality of life. Without the surgery the probability that she will suffer from a stroke, heart disease or diabetes is greatly increased.

The Lap Band surgery IS NOT READILY AVAILABLE THROUGH THE QUEENSLAND PUBLIC HEALTH SYSTEM and I support her application for the funds to be made available from her superannuation.

PART 1.

Following are copies of THREE letters to APRA which have been successful in the EARLY RELEASE OF SUPERANNUATION BENEFITS. Choose whichever letter you or your doctor feel suits your needs best.

Please note that because these letters have been previously successful there is no guarantee that any of them will be successful for your application so please don't blame me if you get rejected. I am merely attempting to provide help to the requests for a copy of a doctor's letter to APRA to be posted on the Superannuation threads.

APRA insists that you include the words life threatening and not readily available through the public health system in your letter.

Good News!!!! I managed to get in an agreement with my Dr's and I don't need to pay everything up- front!!!! I AM GOING TO BE A BANDIT in november!!!

I just called APRA and the guy told me that I can send more additional information for my application and it won't be moved back in line, that would only happen if my application had already gone through the 1st evaluation. So I will be sending my banks statements and my letter today.

The guy also suggested that I would ask for an invoice from My doctor, I called them and the lady said they don't invoice...

The interest free period for the Woolworths Card ceases on the 31 January 2009. Interest charges commence on the 1st February 2009.

Hi Gab,
If I was you I would be keeping my options open. Apply for a credit card (such as the Woolworths one - no interest for six months) as a back up. APRA seem to be quite tough at the moment and with the global financial melt down, they might decide to make it tougher. Hi Gabi,
I included a copy of my bank statement but I believe they actually delve into your bank accounts. The only problem with writing another letter is if you ask for it to be included in your initial application you will start the process all over again. My application went in 09 Sep and I was told on Friday to expect an answer around about 20 Oct - so I assume you would delay your outcome for quite a while.

Assets, investments or money in the bank is something that I don't have.

My GP and my surgeon have written the letters with all the necessary wording "life treating" and "not readily available through Medicare". I have sent the estimates from my doctor and the Anesthetist .

One thing is worrying me, the letter was written by Dr Mosse but the estimate and the doctor who is going to perform the surgery is Dr Gananadha. Do you guys think that would be a major problem?

Also, when I was reading my application again I realized that my statements of why I needed the surgery were not that convincing, so I am writing another letter to them talking about my depression and high cholesterol. I am also going to add my bank statement to prove that I don’t have a penny in my name. Is that a good idea?

Oh by the way, thanks a lot of the help

Superannuation fund you are in doesn't approve withdrawal for surgery, not every single fund does. You could ring your superannuation fund and ask them if they approve withdrawals for life threatening surgery.

Too many assets or too much money in bank or investments.

Regardless of what you hear regarding the 5th, I would ring Apra up frequently and ask when I can expect to be notified of approval or rejection and remind them that surgery is scheduled for the 6th. I'd also write down what is said to me and the name of the person I spoke to.

What are the reasons that APRA can use to deny the funds?

I am so scared of being denied. My surgery will be on the 6th November, in Canberra and I hear that APRA will only release some info on the 5th. I applied for a loan but I am not sure if I will be approved (changed jobs recently and have some debts :/) Does anyone have another suggestion? Because right now APRA is my only hope...

And why are people rejected?

Contact the superannuation company and ask do you have to go through APRA to arrange for funds to be released on 'specified grounds' to have surgery 'to treat a life threatening illness'. The medical profession regard obesity as a 'life threatening illness' some companies don't require you going via APRA.

If you have to go through APRA look up their website and it will tell you what to do, I'll put the website below. Click on SUPERANNUATION and then click on EARLY RELEASE OF SUPER.

The letters from the Surgeon and General Practitioner to APRA MUST HAVE WRITTEN ON THEM 'to treat a life threatening illness' and 'not readily available through the Public Health System'.

Some people who have surgery before APRA has approved it usually pay upfront for their surgery and then put in a claim to APRA (Australian Prudential Regulation Authority), however there is a possibility it will not be approved so I wouldn't suggest anyone do that unless they can afford to live without being reimbursed for the surgery.

Just a heads up ladies and gents- if you are thinking of applying for super release through APRA, they are now looking at an answer by 5/11/08 for applications lodged on or before 03/10/08. OUCH

Hi there?

did any one using APRA have any issues!? As, the reason i am applying is due to not really having a quality of life, and i am infertile as a result of my weight.

Do you think that this will be approved?

And did any one have any problems getting their Dr's to write letters? As i am so worried that my drs will not do this for me, as my BMI is only at around 38!

Sorry, I know im a worry wart, i just like to know what i am up against!

I believe as it is your husbands super that he has to be the one to make the application to apra and claim you as dependent. There is another thread around here some where called apra. I'll try and bump it for you to have a read of as well. I have been having trouble with my surgeons letter too. He has had several clients use the same letter however it wasnt good enough for them this time, neither was the second but the third was fine. However now the problem is hte quotes are too old they tell me. Another call to my surgeons long suffering seceratary to beg her to fax thru a copy with a current date and amount still owing. GOD BLESS her and him!!! Now am just waiting to hear back. Should be tomorrow. My surgery is scheduled for next week. Holidays booked etc...
Now all I need is the money please.... lol

Hi all...

This may have been addressed before.. but I can't find it :(

If I am the one going for surgery - yet I would like to use my husband's superannuation - how do I go about this??? Do I just nominate his Super on the three spaces they offer on the form? Or does HE have to fill out the application and class me as his dependant???

I don't know :(
